Health does not start in a hospital or a clinical but rather in our homes, schools, workplaces, and communities. Induvial healthcare is determined in part, by access to social and economic opportunities as well as the resources and supports available in the home, neighborhood, and community. As part of a movement across the United States and beyond, CrossTx supports Whole Person Care-based initiatives, offering the provisioning of secure health and social services digital networks, supporting care coordination, closed-loop referral management, chronic care management, telehealth, and remote patient monitoring-based initiatives. From connecting emergency rooms and primary care to community supports, such as homeless shelters, food, and transportation services and more, consider CrossTx to support robust Whole Person Care initiatives to help the most at risk of populations.
While the CrossTx platform is accessible in the field, not all provided care and social services are mobile, thus requiring an individual to have or find reliable transportation. Lack of transportation occasionally results in missed appointments, which in turn results in lost revenue. To solve this problem, CrossTx and Uber Health have partnered to provide reliable transportation for these individuals. Clinical care and social service providers request and schedule ride’s for individuals in need of reliable transportation by navigating the same, comfortable user interface of Uber, directly within the CrossTx application.
